Job summary

Yadu Health Aboriginal Corporation is seeking dedicated Aboriginal Health Workers / Practitioners to join our Primary Health Care team in remote communities. Multiple permanent full-time positions are available for those passionate about improving Aboriginal health outcomes with culturally appropriate care.

You will work with a multidisciplinary team to provide holistic care, health education and advocacy, helping clients access services and navigate health systems.

Qualifications

• Certificate IV in Aboriginal and/or Torres Strait Islander Primary Health Care (Practice or Community) or equivalent.
• Experience in a similar health or community-based role.
• Strong understanding of Aboriginal and Torres Strait Islander cultures and communities.
• Knowledge of health issues affecting Aboriginal and Torres Strait Islander people in rural/remote areas.
• Ability to work independently and as part of a multidisciplinary team.
• High computer literacy and ability to maintain accurate client records.

Responsibilities

• Support clients to access culturally safe primary health care and health education.
• Undertake health assessments, screenings and health promotion activities.
• Develop and coordinate individual care for clients using recognised care plans and guidelines.
• Initiate and review health checks, care plans, referrals and recalls.
• Prepare and support clients before and after procedures.
• Build respectful relationships with clients, families and communities.
• Act as a cultural liaison between clients and health professionals.
• Help clients navigate health care systems and connect with services.
• Participate in community meetings, outreach and health promotion activities.
