Job summary

Creative Healthcare Management is seeking a PRN Physical Therapist for weekend coverage at Dukes Memorial Hospital in Peru, Indiana. The PT will develop individualized treatment plans, provide skilled physical therapy, and collaborate with the interdisciplinary team.

The role requires a degree in Physical Therapy, state licensure, strong clinical assessment and communication skills, and the ability to supervise PTAs and work both independently and as part of a team.

Qualifications

• Education: Degree in Physical Therapy from an accredited institution.
• Licensure: Valid Physical Therapist state license or license eligibility.
• Strong clinical assessment and treatment planning capabilities.
• Excellent communication and interpersonal skills; able to work independently and in a team.

Responsibilities

• Resident assessment and treatment to improve safety, mobility and independence.
• Documentation of progress and timely records in EMR.
• Supervision of Physical Therapy Assistants as appropriate.
• Participation in or leadership of team development meetings.
