Job summary

Creative Healthcare Management is seeking a PRN Physical Therapist for weekend coverage at Dukes Memorial Hospital in Peru, Indiana. The PT will develop individualized treatment plans, provide skilled physical therapy, and collaborate with the interdisciplinary team.

The role requires a degree in Physical Therapy, state licensure, strong clinical assessment and communication skills, and the ability to supervise PTAs and work both independently and as part of a team.

Qualifications

• Education: Degree in Physical Therapy from an accredited institution.

Responsibilities

We are seeking a PRN Physical Therapist (PT) for weekend coverage to join our team at Dukes Memorial Hospital in Peru, Indiana. This is a great opportunity for a PT who enjoys working with patients in a hospital setting and is looking for flexible PRN weekend hours.

The Physical Therapist will develop individualized treatment plans, provide skilled physical therapy services, monitor resident progress, and collaborate with the interdisciplinary team. The PT will also provide guidance and supervision to Physical Therapy Assistants (PTAs) as appropriate.

Responsibilities

• Resident Assessment and Treatment: Evaluate residents' functional abilities. Establish and execute a plan of care based on individual goals to improve resident's safety, functional mobility, and overall independence.
• Documentation: Complete timely and accurate documentation.
• Supervision: Provide appropriate guidance and supervision to Physical Therapy Assistants.
• Professional Development: Participate or lead team development meetings.

Requirements

• Education: Degree in Physical Therapy from an accredited institution.
• Licensure: Valid Physical Therapist state license or license eligibility.
• Skills: Strong clinical assessment skills. Proficient in developing and implementing effective treatment plans. Excellent communication and interpersonal skills. Ability to work independently and in a team environment.
• Physical Requirements: Ability to lift and assist clients as needed. Must be able to stand for extended periods and perform physical tasks related to therapy.
• Technology: Computer proficiency to document in electronic medical record platform.
