Job summary

Qualifications

• MD or DO degree with completion of an accredited residency program in Internal Medicine, Family Medicine, or Psychiatry.
• Board certification or eligibility in Geriatric Medicine.
• Minimum of 5-7 years of progressive experience in direct geriatric patient care, preferably in an outpatient or consultative setting.

Responsibilities

• Lead comprehensive assessments of older adults identifying complex medical social and functional needs to develop personalized care plans.
• Provide expert medical management for age-related conditions focusing on optimizing quality of life and minimizing polypharmacy.
• Collaborate with multidisciplinary teams including nurses therapists social workers and pharmacists to ensure coordinated and holistic patient care.
• Develop and implement strategies to prevent and manage common geriatric syndromes like falls delirium and incontinence.
