Qualifications

• Must hold a Bachelor's Degree in Nursing with current registration and licence to practise as a registered nurse.
• Postgraduate qualification relevant to the clinical pathway (e.g., cancer nursing, advanced assessment) is preferred.
• Five years post-registration nursing experience, with at least two years in the relevant specialty.
• CCM credential or equivalent case management certification is desirable.
• Experience in holistic assessment, care coordination, education, escalation, and working with multidisciplinary teams is required.

Responsibilities

• Receive referrals and perform initial holistic nursing assessment.
• Coordinate investigations and appointments; monitor progress and outcomes.
• Provide structured patient and family education on the care pathway.
• Escalate clinical issues promptly to the responsible clinician.
• Maintain accurate pathway documentation and data tracking.
• Collaborate with MDT and allied services to support care continuity.
• Ensure timely communication of changes in clinical status or pathway delays.
• Support continuous improvement and quality of care through data insights.
