About the Role

Our client, a reputable community health non-profit organization, is looking for a compassionate and dedicated Social Worker to join their team in Mossel Bay, Western Cape . This vital role involves providing comprehensive social support services to individuals, families, and groups within the community, with a particular focus on health-related challenges, including chronic illness management, mental health support, and access to healthcare services. You will play a crucial role in connecting community members with essential resources and advocating for their well-being. The ideal candidate will have a strong understanding of social work principles, excellent counseling skills, and a genuine commitment to serving the vulnerable populations of Mossel Bay .

Key Responsibilities

• Conduct psychosocial assessments to identify client needs and develop individualized care plans.
• Provide direct counseling and support services to individuals, families, and groups facing health and social challenges.
• Facilitate access to healthcare services, including medical appointments, therapy, and support groups.
• Develop and implement community outreach programs focused on health education and disease prevention.

Requirements

• A Bachelor of Social Work (BSW) degree or equivalent qualification from an accredited institution.
