Qualifications

• Excellent interpersonal and communication skills, with the ability to engage effectively with diverse populations.
• Willingness to travel within the Mossel Bay area and surrounding communities.

Responsibilities

• Conduct health needs assessments within the community to identify priority areas.
• Educate individuals and families about disease prevention, healthy lifestyles, and available health services.
• Assist clients in navigating the healthcare system, including making appointments and accessing medical facilities.
• Provide basic health screenings and monitor vital signs as directed by healthcare professionals.
• Facilitate referrals to doctors, clinics, hospitals, and other health and social service providers.
• Support patients with chronic disease management through regular follow-up and adherence counselling.
• Organise and conduct community health workshops and awareness campaigns.
• Collect data on health outcomes and program effectiveness for reporting purposes.
