Placements24 is seeking a Social Care Assistant to support individuals and families in the Tembisa community in Gauteng. You will empower clients toward personal goals, provide practical and emotional support, and promote independence in person-centered care.

The role spans community settings with some remote collaboration. You will perform checks, assist with personal care, maintain records, and liaise with families and professionals to ensure coordinated services while adapting to client

About the Role

Our client is seeking an enthusiastic Social Care Assistant to support individuals and families in the Tembisa community. You will play a crucial role in empowering clients to achieve their personal goals and improve their quality of life. This position requires a empathetic individual who can offer practical assistance and emotional support, fostering a sense of belonging and independence. Key Responsibilities

• Assist clients with personal care, medication reminders, and maintaining a safe home environment.
• Conduct regular check-ins with clients, assessing their well-being and identifying any emergent needs.
• Support clients in participating in social, recreational, and educational activities within the community.
• Liaise with families, caregivers, and other professionals to ensure coordinated and effective care delivery.
• Maintain accurate and confidential client records, documenting progress and interventions.
• Provide emotional support and a listening ear to clients facing various life challenges.

Requirements

• Minimum of Grade 12 or equivalent qualification. Relevant certifications in caregiving or social work are advantageous.
• Previous experience in a direct care or support role is essential.
• Knowledge of issues affecting vulnerable populations, such as elderly, disabled, or disadvantaged individuals.
• Excellent communication and listening skills, with a patient and understanding demeanor.
• Ability to adapt to changing client needs and work schedules, including some remote collaboration.
• Proficiency in basic computer skills for record-keeping and communication.
