Placements24 is seeking an experienced Geriatric Care Manager for our Centurion, Gauteng-based client in the Community & Social Care sector. • Minimum of 5 years of experience in geriatric care management.
• Proven knowledge of aging processes, geriatric conditions, and community resources in Centurion.
• Strong assessment, care planning, and problem-solving skills.
• Excellent communication, negotiation, and interpersonal abilities.

Responsibilities

• Conduct thorough assessments of geriatric clients’ needs.
• Develop and implement individualized care plans with clients, families, and healthcare providers.
• Coordinate services including in-home care, medical appointments, and transportation.
• Liaise with physicians and healthcare professionals to ensure seamless care.
• Monitor progress and adjust care plans for optimal outcomes.
• Provide information and support to families regarding eldercare options.
• Advocate for clients’ rights and preferences in all aspects of care.
• Manage crises and respond effectively to emergencies.
• Maintain confidential client records and prepare regular reports.
