Job summary

Placements24 seeks an experienced Rehabilitation Counselor to deliver remote recovery support for clients in Vereeniging and surrounding Gauteng communities.

You will assess needs, develop personalized rehabilitation plans addressing physical, emotional, and vocational aspects, and guide clients through tele-health sessions. The role emphasizes empathy, remote rapport-building, and collaboration with families and local resources to promote independence and quality of life.

Qualifications

• Master's degree in Rehabilitation Counseling or a related field.
• Professional certification or licensure as a Rehabilitation Counselor.
• Minimum of 5 years of experience in providing rehabilitation services.
• Demonstrated ability to conduct effective client assessments and develop treatment plans remotely.
• Proficiency with telehealth platforms, EMR systems, and virtual communication tools.
• Strong knowledge of disability services and resources available in Gauteng and surrounding areas.

Responsibilities

• Conduct comprehensive remote assessments of clients' physical, psychological, and vocational needs.
• Develop individualized rehabilitation plans to support client recovery and independence.
• Provide counseling and support to clients and their families throughout the rehabilitation process.
• Assist clients in identifying and accessing appropriate community resources, support groups, and vocational training.
• Monitor client progress and adjust rehabilitation plans as needed via virtual check-ins.
• Educate clients on strategies for managing their condition and promoting self-care.
