You will collaborate with clients and families via virtual platforms, creating personalized plans and connecting them with essential services and opportunities. The role requires empathy, remote engagement skills, and strong problem-solving abilities.

Qualifications

• Degree or certification in Social Work, Psychology, Disability Studies, or related field.
• Experience supporting individuals with various disabilities.
• Proficiency with remote communication technologies and case management software.
• Strong advocacy, disability rights knowledge, and excellent communication.
• Ability to work independently and manage a remote caseload.

Responsibilities

• Conduct remote assessments to understand client needs and goals.
• Develop and implement individualized support plans and coordinate services.
• Provide guidance and advocacy for clients navigating disability services and benefits.
• Facilitate virtual connections to community resources and support networks.
• Maintain confidential client records and monitor progress toward goals.
• Educate clients and families on rights, resources, and self-advocacy.
