Job summary

Placements24 is seeking a dedicated Remote Case Manager to support individuals with addiction recovery from home. You will assess needs, develop treatment plans, and coordinate care with healthcare professionals to ensure integrated support for clients in Durban and beyond.

The ideal candidate holds a Bachelor's degree in Social Work, Psychology, Counseling, or a related field with professional registration, and has at least two years of addiction counseling or case management experience.

Qualifications

• Bachelor's degree in Social Work, Psychology, Counseling, or related field.
• Professional registration with SACSSP or HPCSA.
• At least 2 years' experience in addiction counseling or case management.
• Experience with virtual platforms and EHR systems; strong empathy and advocacy.

Responsibilities

• Conduct comprehensive assessments of clients' needs related to addiction and co-occurring mental health issues.
• Develop, implement, and monitor personalized treatment and recovery plans.
• Coordinate client care with physicians, therapists, and other healthcare professionals to ensure integrated support.
• Provide ongoing counseling, crisis intervention, and support to clients and their families via virtual platforms.
• Assist clients in accessing resources such as housing, employment, and support groups.
• Maintain accurate and confidential client records in accordance with ethical and legal standards.
