Job summary

Yee Hong Centre for Geriatric Care in Toronto is seeking a Part-Time Program Coordinator for Home Support Services to coordinate home care, meals and transportation, and to provide timely, professional intervention to clients and families.

You will monitor health status, update records, coach staff, schedule resources, and ensure compliance with risk management policies while helping seniors remain independent and safe at home.

Qualifications

• Bachelor's degree in social services, health care, activation, or equivalent.
• Minimum 2 years experience in social services or health care; supervisory experience is an asset.
• Knowledge of working with seniors.

Responsibilities

• Coordinates delivery of home support services to enable clients to age in place.
• Coordinates meal and transportation services for clients.
• Provides timely and professional client intervention to clients and family.

Program Coordinator, Home Support Services (Part-Time)

Responsibilities

• Coordinates the delivery of appropriate home support services to enable clients to age in place
• Coordinates meal and transportation services for clients
• Provides timely and professional client intervention to clients and family
• Monitors overall health status and keeps up-to-date health records of clients
• Coaches, develops and evaluates staff to ensure they meet their required performance standard. Accurately schedules staff resources in accordance with clients' needs as reflected in the care plan
• Ensures implementation of risk management policy and safe environment for clients and staff
• In consultation with the Manager, develops and updates policies, procedures for the program
• Coordinates the optimal deployment of volunteers for the program and provides effective guidance, training and support to the volunteers
• Maintains up to date and accurate files, records and statistics on the program
• Ensures efficient and responsible use of resources and maintains accurate record of fees and expenditures
• Provides emergency response to emergency calls from staff and clients
• Effectively promotes the home support services to clients of the building/neighbourhood
• Keeps up-to-date with developments and best practices in the field
• Other duties as assigned

Qualifications

• Bachelor's degree in social services, health care, activation, or equivalent
• Minimum 2 years experience in social services or health care, supervisory experience an asset
• Knowledge and experience in working with seniors
• Computer proficiency, with knowledge of assessment software, AlayaCare, and Microsoft Office Solutions
• Current CPR Certificate required
• Satisfactory Police Reference Check (with vulnerable sector screen) result
• Language Requirement: Ability to communicate in English and Chinese

• Occasional travel
• Use of own vehicle

Salary Range

$34.28 - $40.32
