Placements24 is seeking a compassionate Rehabilitation Support Worker in Polokwane, Limpopo. You will work closely with therapists and case managers to deliver personal care, implement rehabilitation plans, and promote independence within the local community.

The role focuses on direct client interaction, safety, and collaboration with a multidisciplinary team to help clients reach their personal goals and improve quality of life in the Polokwane area.

Qualifications

• Relevant qualification or certification in healthcare support, disability care, or related field.
• Minimum of 2 years of experience as a support worker, care assistant, or similar role.
• Experience with individuals with physical or intellectual disabilities is essential.
• Understanding of rehabilitation principles and person-centered care.
• Excellent practical skills in providing personal care and support.
• Strong communication and interpersonal abilities.
• Patience, empathy and a compassionate attitude.
• Ability to follow instructions and work in a multidisciplinary team.
• Basic literacy and numeracy for documentation.

Responsibilities

• Provide direct personal care and support to clients with varying needs.
• Assist with daily living activities including hygiene, mobility, and meals.
• Implement rehabilitation and therapy programs as directed by healthcare professionals.
• Support client engagement in community activities and social participation.
• Monitor progress, well-being, and behavior; report changes to supervisor.
• Maintain safe, clean living environment and confidentiality of records.
• Administer medications as prescribed and monitor for side effects.
• Assist with transportation to appointments and engagements.
• Foster independence, dignity and self-esteem.
• Communicate effectively with clients, families and the team.
• Adhere to organizational policies and professional standards.
