  • Facilitate palliative care needs rounds and multidisciplinary discussions.
  • Partner with GPs, specialist providers and multidisciplinary teams to support quality resident outcomes.
  • Promote advance care planning and resident‑centred decision making.
  • Provide support and guidance to teams caring for residents with complex symptoms and changing care needs.
  • Contribute to quality improvement initiatives, projects and service development opportunities across the organisation.

  • Nurse Practitioner applicants must hold current NP endorsement and be able to practise autonomously within their scope of practice.
  • Demonstrated ability to provide specialist clinical leadership, consultation and support.
  • A passion for mentoring, coaching and developing others.
  • Exceptional interpersonal and relationship‑building skills.
  • The ability to navigate complex clinical situations with professionalism, empathy and sound judgement.
  • A collaborative approach and the ability to build credibility with stakeholders at all levels.
  • Confidence working independently while recognising the value of partnership, communication and shared accountability.
