Job summary

Placements24 is seeking a dedicated Social Worker focused on Community Outreach to serve the Mossel Bay area. You will connect with individuals and families in need, provide essential support services, resource navigation, and advocacy to drive positive social change.

The role involves needs assessments, developing intervention plans, counseling, facilitating access to housing, healthcare, employment, and legal aid, advocating for clients, and building outreach programs to engage underserved

Qualifications

Registered Social Worker qualification and professional registration.
Proven experience in community-based social work or case management.
Strong understanding of social welfare systems and community resources.
Excellent assessment, counseling, and intervention skills.

Responsibilities

Conduct needs assessments and develop comprehensive social support plans for individuals and families.
Provide counseling and support services to clients facing social, emotional, or economic challenges.
Facilitate access to community resources, including housing, healthcare, employment, and legal aid.
Advocate for clients' rights and needs within various systems and agencies.
Develop and implement outreach programs to engage underserved populations.
