Empower Healthcare is seeking a qualified Physiotherapist to deliver home-based physiotherapy across NDIS participants, focusing on Musculoskeletal, Neurological, and Cardiorespiratory care. You will work within a supportive team to provide comprehensive services in participants' homes.

Qualifications

• Bachelor of Physiotherapy (or equivalent) is required.
• Excellent verbal and written communication skills are essential.
• Strong time-management and attention to detail are required.

• Work holistically with other allied health professionals to provide a multidisciplinary care plan.

• Liaise with service providers to ensure effective communication and coordination.

The Position

Our innovative service model focuses on delivering the best clinical outcomes for participants. As a Physiotherapist with Empower, you will have the opportunity to work with a variety of Musculoskeletal Neurological and Cardiorespiratory patients.

Working in a supportive team environment, you will be responsible for the delivery of Empower Healthcare's comprehensive services across Support at Home and NDIS participants in Sutherland and St George.

You will:

• Conduct comprehensive assessments and deliver tailored physiotherapy treatment plans for participants in their own home through Support at home packages/NDIS.
• Work holistically with other allied health professionals to provide a multidisciplinary care plan
• Educate participants and their families to optimise their ability to live healthy lives within the community
• Liaise with service providers to ensure effective communication
• Work autonomously to prioritize and complete tasks, maintain records and provide patient reports

You will be provided with support from our home care team who will schedule these appointments and plan an efficient route.

You must have:

• Bachelor of Physiotherapy (or equivalent)
• Registration with AHPRA
• Police Check Clearance
