Placements24 is seeking an Elderly Care Specialist to support seniors remotely in Mossel Bay and surrounding areas. You will assess needs, coordinate services, provide emotional support, and advocate for clients using telecommunication tools.

The role emphasizes personalized care plans, collaboration with families and healthcare providers, and monitoring progress to adapt plans as required. A compassionate, communicative professional with experience in gerontology or nursing is preferred.

Qualifications

Relevant qualification in Gerontology, Social Work, Nursing, or a related field.
Experience in elderly care, case management, or a similar support role.
Strong understanding of the challenges and needs associated with aging.
Excellent communication and interpersonal skills, with the ability to build rapport virtually.
Proficiency with remote communication tools and technology.

Responsibilities

Conduct virtual needs assessments for elderly clients, including physical, emotional, and social well-being.
Develop and coordinate individualized care plans and support services.
Provide emotional support, companionship, and engagement through virtual means.
Liaise with families, healthcare providers, and community resources to ensure seamless care coordination.
