Qualifications

Responsibilities

• Provide direct support and case management services to individuals recovering from substance abuse.
• Assist clients in developing personalized recovery plans and setting achievable goals.
• Connect clients with community resources such as rehabilitation centers, counseling, employment support, and housing assistance.
• Facilitate group support sessions and educational workshops on addiction and recovery topics.
• Monitor client progress, document interventions, and maintain records.
• Offer emotional support and encouragement to clients and their families.
• Conduct outreach activities to engage individuals in need of support services.
• Collaborate with service providers and community stakeholders to ensure comprehensive care.
• Advocate for clients' needs and rights within the community and service systems.
