Placements24 is seeking a proactive Community Health Advocate to join the Community & Social Care team in Polokwane, Limpopo. The role focuses on connecting residents with vital health and social services, conducting needs assessments, and facilitating referrals to ensure equitable access to care.

The ideal candidate has a health sciences or related degree, strong communication and listening skills, and the ability to work independently while managing multiple tasks in a supportive team

Qualifications

• Diploma or Degree in Health Sciences, Social Work, Community Development or a related field.
• Experience in community health, social work, or a related advocacy role is required.
• Strong knowledge of the local health and social service landscape in Polokwane.

Responsibilities

• Engage with community members to identify health and social service needs.

• Assist individuals and families in navigating healthcare and social service systems to access necessary care.
