Job summary

Placements24 is seeking a Community Health Program Facilitator to design and deliver health education sessions and workshops in Mossel Bay, Western Cape. You will connect residents with local services and promote healthy lifestyles through engaging programming.

This role blends on-site facilitation with administrative duties and requires at least 2 years of experience in community health and a relevant qualification.

Qualifications

• Minimum 2 years of experience in facilitating health programs.
• Experience in community-based settings preferred.
• Ability to develop engaging health education content.
• Proficiency with basic computer skills (Word, email).
• Ability to work independently and as part of a team.

Responsibilities

• Develop and deliver health education sessions and workshops on nutrition, hygiene, disease prevention, and reproductive health.
• Conduct needs assessments to identify priority health issues and tailor content.
• Facilitate group discussions and activities to encourage community participation.
• Connect community members with local healthcare providers, clinics, and services.
• Create and distribute accessible health information materials.
• Build relationships with community leaders and health workers.
• Track attendance and gather feedback to evaluate program effectiveness.
• Organize health awareness campaigns and outreach events in Mossel Bay.
• Maintain records of activities, demographics, and health outcomes.
