Higher Health is seeking a Disability Student Support and Case Management professional to provide psychosocial support to students with disabilities, assess cases, implement Individual Support Plans, and collaborate with a multidisciplinary team to ensure timely care.

The role includes disability screening, advocacy for reasonable accommodation, stakeholder engagement, and ongoing monitoring of outcomes to boost student retention and success at campus level.

Qualifikationen

• Bachelor’s degree in social work (essential).
• Current SACSSP registration as a Social Worker (essential).
• Experience providing psychosocial support and case management for students with disabilities.

Aufgaben

• Provide psychosocial support and counselling to students with disabilities.
• Assess and support cases referred through the case management process.
• Implement and review Individual Support Plans for identified students.
• Coordinate multidisciplinary meetings and monitor referral outcomes.
• Advocate for reasonable accommodation and inclusive practices.
