The A Career is seeking a Family Coordinator to join as a full-time team member. You will manage newly authorized client cases from confirmation through early service stabilization, guiding families and providers through the start-of-care period.

Responsibilities include onboarding families, coordinating assessments and staffing, ensuring schedules, and maintaining timelines. This role emphasizes proactive problem-solving and clear communication with multiple stakeholders for successful service

Formación

• Advanced English required.
• Customer service skills to handle caregiver/patient/insurance interactions.
• Strong organizational skills for documentation and coordination.
• Excellent cross-functional communication across stakeholders.

Responsabilidades

• Serve as the primary contact for families during service initiation.
• Guide families through onboarding steps, timelines, and expectations.
• Coordinate assessments, provider assignment, and service start.
• Ensure services are scheduled and providers are ready to begin.
• Track early service delivery and resolve issues affecting consistency.
• Collaborate with internal teams to maintain timelines and readiness.
• Coordinate transitions of stable cases to Case Management.
