Job summary

Qualifications

  • High School Diploma or GED; and three years’ experience in peer related services, case management, substance abuse, corrections, law enforcement, or behavioral health related tasks.

Responsibilities

  • Attend necessary staff meetings to identify and intervene with violence-impacted individuals and refer to appropriate services for resources.
  • Accompany School Based VIP team on interventions with individuals at high risk to be involved in cycles of violence, gang involvement, weapons use, or substance abuse.
  • Develop culturally specific, client-centered case management plans that utilize trauma-informed resources to support program participants and their families.
  • Provide intensive case management according to school-based diversion strategies and higher-level management staff.
  • Maintain case management documentation in a timely, thorough, and accurate manner and ensure confidentiality per FERPA guidelines.
