Job summary

Rite of Passage is hiring a Transitional Care Coordinator for the Southern California Treatment Program in Torrance, CA. The role focuses on administering and coordinating case management services for youth, acting as a liaison among residents, families, and agency staff, and guiding youths back into the community.

Shifts are Monday–Friday, 9:00 am–6:00 pm. Benefits include medical insurance after 60 days, 403b match after 1 year, and PTO accrual.

Qualifications

• Associates degree in a related field with at least one year of experience working with at-risk juveniles and their families or other related experience.
• Bachelor’s degree preferred.
• Hold a current State Driver’s License with an acceptable driving record for the past three years.

Responsibilities

• Administer, develop, and facilitate the completion of case management services for assigned residents.
• Serve as liaison between residents and site departments, parents/guardians, and placing agency officials to support transitions.
• Ensure successful transition of youth back to the community.
