Job summary

BCFS Health and Human Services in Texas is seeking a Transition Specialist to provide quality case management services to youth aging out of foster care, guiding them toward goals with professional judgment and best practices.

You will assess personal needs, help obtain resources, advocate across human services, health, and court systems, and develop Individual Service Plans with youth, updating progress regularly.

Qualifications

• Bachelor’s degree in a social science field required.
• 2 years of experience in case management, job development or crises intervention with youth required.
• Experience in community building, event coordination and public speaking preferred.

Responsibilities

• Assess the personal needs of PAL clients and help obtain resources by providing case management services.
• Advocate with human services, health, and court and criminal justice systems to resolve client issues.
• Assist youth in crisis by de-escalating situations and developing plans to move out of state crises.
• Develop detailed Individual Service Plans for each client and review progress regularly.
• Maintain timely and accurate file documentation and data entry meeting PAL program requirements.
• Follow BCFS HHS guidelines and policies for PAL TFSS services.
• Demonstrate knowledge of contracts and agency policies and procedures.
