Job summary

BCFS Health and Human Services is seeking a Transition Specialist in San Antonio, TX. You will provide case management for youth aging out of foster care, helping them achieve goals and access resources.

The role requires a Bachelor's degree in a related field and 2 years of experience in youth services, with strong skills in advocacy, crisis intervention, and public speaking. Evening/weekend work and travel may be required.

Qualifications

• Bachelor's degree in Human Services, Social Work, Education or related Social Science field.
• Minimum 2 years of experience in case management, job development or crises intervention with youth; experience in community building, event coordination and public speaking.
• First Aid and CPR certificates required.
• Valid Texas Driver's license with clean driving record and reliable transportation.
• Pass pre-employment drug screen and background checks.

Responsibilities

• Assess PAL clients' personal needs and link to resources.
• Advocate with human services, health, court and criminal justice systems.
• Assist youth in crisis with de-escalation and planning to move out of state crises.
• Develop detailed Individual Service Plans and monitor progress.
• Maintain timely file documentation and data entry per PAL program.
• Follow BCFS HHS policies and PAL TFSS guidelines.
• Understand contracts and agency policies.
• Participate in weekly and monthly PAL program events.
• Work evenings or weekends as needed.
• Ensure safety protocols during emergencies.
• Obtain service approvals via form 2054.
• Know community resources.
• Maintain confidentiality.
• Travel as needed.
