Youth Opportunity Investments, LLC seeks a Transition Services Manager at Ocala Youth Academy to guide youth from residential care back into the community. You will manage cases, coordinate with families, and align services with treatment plans to support successful reintegration.

The role requires strong case management, knowledge of juvenile services, and the ability to partner with probation, schools, and community resources. Full-time position with benefits and competitive pay.

Qualifications

• Bachelor’s degree in counseling, social work, psychology, rehabilitation, special education, or related human services field preferred.
• Experience with the state Department of Juvenile Justice is a plus.

Responsibilities

• Provide case management services to youth and families during transition.
• Assist youth with employment, education, training, and career planning.
• Coordinate with treatment teams, families, and community agencies for a smooth transition back to the community.
• Develop and organize comprehensive transition plans including housing, transportation, and documents.
• Communicate with external community sources to support engagement.
• Develop individualized program plans and life skills for youth reintegration.
• Oversee transition processes and ensure timely service delivery.
• Act as liaison between families and state agencies as needed.
