Qualifications

• Bachelor’s degree required.
• 2 years experience in treatment plan development, community referral coordination, service plan development, collaboration with various agencies, resource development and effective utilization.

Responsibilities

• Maintain active network with community agencies to identify resources, make referrals, and track progress.
• Review CAT outcomes and recommend evidenced based delinquency interventions.
• Ensure youth are connected to outside mental health, substance abuse, and sex offender services as needed.
• Follow up monthly with service providers to verify participation and engagement.
• Serve as liaison for communication to formal providers and Aftercare Navigator.
