Job summary

Socket.dev is seeking a Resource Navigator to manage transitional care for youth exiting residential programs in Florida. You will coordinate with the JPO, RCM, and families to identify needs, map local resources, and guide transition plans from assignment through discharge.

You will participate in conferences, monitor referrals, and ensure all steps align with YES Plan goals. Strong communication, organization, and HIPAA knowledge are essential for successful outcomes.

Qualifications

• Bachelor's Degree in related area of study.
• 2 years of experience in treatment plan development, community referral coordination, and service plan development.
• Experience collaborating with multiple agencies and resource development.

Responsibilities

• Review case information and JJIS data to plan transitions.
• Coordinate with JPO, RCM, youth, and family to identify needs and referrals.
• Develop Community Asset Mapping to locate local resources near youth.
• Lead Transition Conference and CRT discussions with identified services and referrals.
• Ensure timely communication with parents/guardians and providers throughout the process.
• Maintain regular contact with JPO and CRT/Exit Conference teams to track referrals.
• Prepare YES Plan and discharge-related documentation after program completion.
