Job summary

Molina Healthcare of Florida is seeking a Community Connector, Transition Resource Specialist to support members transitioning from pediatric to adult health care. You will work with Care Managers to address barriers, provide education and connect members to community-based resources, housing, transportation, and social services.

The role involves system navigation, outreach, and collaboration with the health care services team to maximize member health plan benefits.

Qualifications

• At least 1 year of health care experience with underserved or special needs populations.
• CHW certification may be required for certain states.
• Valid and unrestricted driver's license and reliable transportation.
• Ability to multi-task while engaging with members.
• Excellent customer service, organization, and time-management skills.
• Willingness to learn other programs and software systems.

Responsibilities

• Serve as subject matter expert for Transition-Aged Youth resources, programs and best practices.
• Collaborate with Care Managers to support members moving from pediatric to adult healthcare systems.
• Provide education, consultation and guidance to members, families and care teams on transition planning.
• Identify and connect members to community resources including health, education, vocational, and social supports.
• Assist with system navigation and help members access adult services.
• Support transition readiness and planning activities and monitor outcomes.
• Engage with members to assist in managing health care needs and benefits.
