Job summary

BCFS Health and Human Services seeks a Lead Transition Specialist to ensure quality transition services for youth, using best practices and compliant with program standards. You will guide staff, manage casework, and oversee client progress in crisis situations.

Responsibilities include developing Individual Service Plans, coordinating with DFPS PAL, training staff, and reporting program outcomes. Strong collaboration with leadership and field offices is essential.

Qualifications

• Bachelor's degree in social services or related field required.
• 2 years of experience in case management, job development or crises intervention with youth required.
• Experience in community building, event coordination and public speaking preferred.

Responsibilities

• Provide case management to young adults 16-21 eligible for the program.
• Develop Individual Service Plans, assess progress, maintain client files.
• Obtain DFPS youth participation approval (2054) from PAL staff.
• Follow DFPS PAL guidelines and policies.
• Lead and mentor direct reporting staff to promote performance.
• Ensure staff receive necessary training for high-quality programming.
• Attend required meetings and conferences; provide crisis management.
• Make referrals to outside agencies and provide follow-up.
• Present at conferences on transition services.
• Maintain client confidentiality.
