Qualifications

Responsibilities

• Assist eligible participants with self-sufficiency plans including education and employment.
• Review and update individualized service plans and case notes.
• Assist participants in accessing community resources and housing services.
• Develop relationships with institutions to coordinate services for participants.
• Maintain documentation for TWC grant in line with guidelines.
• Travel within multi-county region to deliver services.
• Represent Buckner to external agencies and promote the FYI program.
