Job summary

Buckner International is seeking a Transitional Services Case Manager Aftercare to provide case management for foster care youth transitioning to adulthood and alumni pursuing self-sufficiency. The position requires a bachelor's degree in social services or a related field and at least one year of casework experience.

Qualifications

• Bachelor's degree in social services or related field required.
• Minimum 1 year of casework experience in youth services preferred.
• Experience delivering services to foster youth or young adults is a plus.

Responsibilities

• Assist eligible participants with creating self-sufficiency plans covering education, vocational training, employment and life skills.
• Provide case management including reviewing and updating individualized service plans, case notes, and referrals.
• Help participants access community resources, enroll in programs, obtain housing and other services.
• Develop relationships with education, training, housing and child care providers to coordinate services.
• Contribute to development and delivery of independent living skills curriculum.
• Travel within a multi-county region to serve clients and improve program delivery.
• Complete documentation for TWC grant accurately and on time.
• Stay up to date with TWC services to assist foster youth with job searches and online profiles.
