Job summary

First Place for Youth in Oakland, CA is seeking a Direct Care Floater to provide direct youth support, intensive onsite and community-based case management, and resource referral for Transition Age Youth in our Rapid Re‑housing program.

The role requires on-site and community availability three days per week, working with youth facing homelessness to identify goals in self-sufficiency, education, employment, housing, and personal advocacy.

Qualifications

• Bachelor Degree in Social Welfare, Psychology, or equivalent required
• At least three years’ experience working with homeless, at‑risk, transition age (18–24‑year‑old) or foster youth populations with complex presentations including presentations of chronic trauma.
• Must be comfortable working in community settings.
• Experience working with participants who have mental health and substance abuse issues
• Must have a car, a valid CA driver’s license and auto insurance.

Responsibilities

• Provide case-management and skills development to participants in the program.
• Conduct monthly meetings with participants to identify and accomplish goals.
• Create and deliver service plans focusing on wellness, housing, education, employment, and independent living.
• Coordinate with referral agencies and track progress; develop ways for participants to practice independence.
• Assist housing stability by case management; help participants move into supported housing and locate resources.
• Ensure apartment cleanliness through monthly inspections and provide resources to residents.
• Support partnership with property management; attend meetings and coordinate events/workshops.
• Maintain caseload files and ensure timely documentation of services in data system.
• Collaborate with colleagues across the organization and perform other duties as assigned.
