Job summary

Marie Detty Youth and Family Services in Lawton, OK is seeking a full-time Case Manager to support residents through education, employment and personal development planning. You will coordinate services with schools and families, ensure accurate documentation, and participate in safety and behavior programs.

Minimum qualifications include a Bachelor’s degree in Psychology, Social Work or Counseling and state certification or two years of related experience.

Qualifications

  • Must have a valid driver’s license and telephone.
  • Must be able to lift at least 25 lbs.
  • Must be able to physically restrain residents if necessary (training provided).
  • Must pass FBI background investigation.
  • Ability to communicate with and advocate on behalf of residents.
  • Ability to maintain records and confidentiality.
  • Must attend mandatory trainings and monthly staff meetings; START training within 90 days; CPR/First Aid within 90 days.

  • Assist in developing transition plans for residents leaving care.
  • Develop partner relationships with other community organizations.
  • Conduct outreach and coordinate community engagement activities.
  • Act as the facility’s Grievance Coordinator.
  • Attend treatment team and transition planning meetings; help with shift coverage.
  • Attend monthly administrative team and staff meetings; participate in on-call rotation.
  • Work with multidisciplinary team to ensure quality resident care.
  • Complete minimum training hours per year (24 hours full-time; 12 hours part-time).

Responsibilities
  • Help and support residents with concerning issues that may affect them.

  • Complete the basic living skills, behavior and behavior supports, educational services, employment services, social skills, and successful adulthood components of the resident’s initial individual master treatment plan and MTP reviews, documenting discussion of the plan with the resident, caseworker, and family as appropriate.
  • Assure case files contain accurate and complete documentation.
  • Maintain and oversee the youth advisory council to integrate youth voice into program design, policy and procedure, and implementation.
  • Conduct monthly youth advisory council meetings.
  • Assist in implementing the behavior management system.
  • Assist in developing transition plans for residents leaving care.
  • Develop partner relationships with other community organizations.

  • Practices person-centered care and uses active listening skills. Expresses empathy and care when communicating with clients. Engages youth in asset building activities and encourages empowerment.
  • Focus on youth, family, and community involvement in all aspects of the treatment process.
  • Implements a robust successful adulthood program to include peer mentorship opportunities, independent living skills training, and community partnerships.
  • Identifies potential safety issues and takes measures to reduce risks and protect clients and staff.

  • Adhere to all regulations concerning confidentiality, judgment, tact, and diplomacy in contacts with persons served, parents/guardians, case workers, and local government agencies.
  • Ability to assess situations, identify potential safety issues, and independently initiate safe interventions.
  • Must be able to attend mandatory trainings and monthly staff meetings as scheduled. The Case Manager is required to participate in START training (within 90 days) and pass the written and physical skills test of Managing Aggressive Behavior (within 30 days) of employment. The Case Manager is also required to become certified (within 90 days) in Medication Administration and CPR/First Aid.

This is a full time 40-hour per week hourly position classified as eligible for overtime according to the provisions of the Fair Labor Standards Act. Marie Detty Youth and Family Services is an at-will employer. Employment is by mutual consent of the company and employee and either may terminate the relationship at any time, with or without cause or advance notice.
