Job summary

Institute for Health and Recovery (IHR) is seeking a Clinical Case Manager to work with youth in the Malden TIP program, bridging school-based services with community supports. The role requires a Master’s degree and experience with at-risk youth, families, and trauma-informed practice.

You will collaborate with school staff, DCF, and community partners, delivering home and community-based services, and coordinating individualized plans for youth and families in a hybrid onsite/remote

Qualifications

• Master’s degree in social work, counseling, psychology or related field, required.
• Experience with adolescents/transition-age youth and families.
• Experience providing treatment at home, school, or in community settings.

Responsibilities

• Provide clinical case management to youth at Malden Salemwood School.
• Deliver strengths-based services to youth involved with DCF.
• Conduct intakes and needs assessments for program participants.
• Coordinate care with families, school staff, and community partners.
• Develop, implement, and review Individual Service Plans.
• Lead afterschool and summer activities for participants and families.
• Maintain written reports and records including electronic health records.
