Job summary

Burlington United Methodist Family Services, Inc. d/b/a Star of Hope Family Services is seeking a compassionate Case Manager to join our Residential Treatment team in Beckley, WV. You will advocate for youth, coordinate services, and support treatment planning in collaboration with families and community partners.

Qualifications

• Bachelor's degree in Social Work, Psychology, Human Services, Counseling, or a closely related field.
• Valid driver's license with an acceptable driving record.
• Ability to successfully complete required background screening.

Responsibilities

• Coordinate case management services for youth receiving residential treatment.
• Develop and maintain supportive relationships with youth and their families.
• Schedule, coordinate, and document Treatment Plan Meetings.
• Conduct regular individual meetings with assigned residents.
• Coordinate services with therapists, physicians, psychologists, schools, juvenile probation, child welfare agencies, and other community partners.
• Complete required assessments, including CAFAS and APS Care Connect, and other clinical documentation.
• Maintain timely, accurate case notes, monthly summaries, home visit documentation, court reports, and treatment records.
• Communicate regularly with the West Virginia Department of Health and Human Resources and other referral agencies.
• Attend multidisciplinary team meetings, court hearings, and MDT meetings as needed.
• Assist with discharge planning and securing appropriate placement options for youth.
