Job summary

The State of South Carolina is seeking a rehabilitative services professional to develop and monitor individualized treatment and case plans based on risk assessments and identified needs. You will conduct counseling across orientation, assessment, individual, group, and family sessions, documenting all services per policy.

You will collaborate with other state agencies, participate in multidisciplinary meetings, reentry planning, and referrals.

Qualifications

• Bachelor's degree in guidance, counseling, or the social sciences with professional human services experience.
• Knowledge of psychology, sociology, human services and therapeutic techniques.
• Strong communication and time-management skills; proficient with basic computer applications.

Responsibilities

• Develops, implements, and monitors individualized treatment and case plans based on risk assessments and identified needs.
• Plans and conducts all assigned rehabilitative counseling services including orientation, assessment, individual, group, and family counseling.
• Documents all offender orientation, assessments, case management, crisis interventions, and other rehabilitative services per policy.
• Works with state agencies, participates in multidisciplinary meetings, classification reviews, and reentry planning; submits monthly reports.
• Assists in transition and reentry, coordinating referrals, educational placements, and aftercare services.
• Must successfully complete four weeks of SCDC Basic Training upon hire.
