Job summary

Qualifications

• Bachelor’s degree in Social Work or related field; Master’s preferred.
• Minimum of one year community-based experience.
• Strong cultural competency and communication skills.
• Bilingual in Spanish and English preferred.
• Proficiency in documentation and highly organized.

Responsibilities

• Offer one-on-one, strength-based support to help youth achieve recovery goals.
• Share recovery knowledge and lived experience to empower youth.
• Assist in crisis planning and develop crisis management strategies.
• Connect youth and families to community resources and natural supports.
• Build collaborative relationships with families, providers, and team members.
• Complete documentation and ensure compliance with standards.
• Participate in professional development, team meetings, and training.
• Fulfill mandated reporting and collaborate within a multidisciplinary team.
