Job summary

Anna Jaques Hospital is seeking a Case Manager for the Student Assistance Program (Gloucester). You will provide school-based prevention services to youth at risk, helping them develop coping skills and access resources within the school and community.

Responsibilities include counseling, prevention education, community outreach, and administrative tasks. Evening hours required, with some weekend outreach; valid driver’s license and reliable vehicle are necessary.

Qualifications

• Bachelor-level credential or higher in counseling, psychology, social work or related field.
• Experience with youth, families and substance use issues.
• Ability to document electronically and maintain records.

Responsibilities

• School Counseling: provide preventative individual/group support under supervision.
• Prevention Education: lead classroom presentations and health campaigns.
• Community Based Services: plan/provide outreach events and transportation as needed.
• Administration: maintain logs, complete paperwork and track outcomes.
• Public Representation: participate in health fairs and community coalitions.
• Other duties as assigned.
