Job summary

AGAPE CHILD AND FAMILY SERVICES INC in Memphis, TN is seeking a full-time Case Manager to support youth and families through individualized service plans. Travel biweekly to Tipton, Lauderdale, and Fayette Counties combines with virtual meetings to sustain engagement and service continuity.

You will connect participants with employment opportunities, educational programs, and community resources while maintaining thorough documentation and collaborating across departments to achieve program

Qualifications

Bachelor's degree in social services, human services, business administration, or a related field is required.
Minimum of two years of relevant experience.
Strong written and verbal communication and problem-solving skills.
Proficiency with Microsoft Office Suite.

Responsibilities

Case Management & Service Planning: develop individualized service plans with youth and families.
Rural Service Delivery & Virtual Engagement: biweekly county rotations and virtual meetings.
Employment & Education Connection: link youth to employment, training, and education opportunities.
Ongoing Case Contact & Documentation: monthly contact and accurate documentation in VOS.
Partnership & Community Engagement: collaborate with schools, employers, training providers, and community partners.
Organizational Engagement: attend meetings and uphold PQI standards.
