Job summary

Qualifications

• 4 years of relevant experience or a Bachelor's degree in a human service field preferred.
• Outreach and case management experience strongly preferred.
• Data collection experience a plus.
• Strong computer and internet skills, including Microsoft Office.

Responsibilities

• Identify goals and track progress for youth ages 10-24.
• Coordinate needed services and provide warm referrals.
• Advocate and facilitate multi-systemic wraparound services.
• Build community connections and work as part of a supportive team.
• Some Evening and Weekend work required.
