Job summary

18 Degrees seeks Outreach Case Workers for its Community Connection program in Pittsfield and North Adams, MA. The role focuses on building trust with youth ages 10-24, coordinating services, and providing advocacy and referrals.

Candidates with lived experience and multilingual fluency are a plus; evening/weekend work may be required. A bachelor's degree in a human service field or equivalent experience is preferred, along with strong data collection and Microsoft Office skills.

Qualifications

• Trauma-informed care and knowledge of gang culture.
• Meet individuals where they are; some evening and weekend work required.

• 4 years of relevant experience or bachelor's degree in a human service field preferred.
• Outreach and case management experience strongly preferred.
• Data collection experience a plus.
• Strong computer and internet skills, including Microsoft Office.

Responsibilities

• Identify goals, strengths and track progress for youth; coordinate needed services; provide warm referrals, individualized support and advocacy.
• Facilitate multi-systemic wraparound services for at-risk youth (ages 10-24) in Pittsfield, MA.
• Collaborate with counseling and mentoring personnel to support youth in the community.

Outreach Case Workers in our Community Connection (PCC) working with youth are focusing on the welfare of our youth, to avoid negative impact and succeed in their futures by identifying their goals; strengths and tracking progress; coordinating needed services; providing warm referrals, individualized support & advocacy; and facilitation groups. Working in both our Pittsfield and North Adams offices.
