Job summary

Durham County Government is seeking a Project BUILD Outreach Worker to engage youth and young adults, ages 10-21, and their families. The role maintains a regular caseload of 15-25 participants, conducts intake assessments, and collaborates with schools, courts, and community partners.

Strong documentation and teamwork are essential. Responsibilities include intensive home visits, service linkage, and facilitating group sessions while maintaining flexible hours to meet program needs.

Qualifications

• Experience working with youth and young adults.
• Knowledge of local gang issues and needs of gang involved and high-risk youth.
• Ability to work effectively with high risk and gang involved youth/young adults and their families.

Responsibilities

• Work with youth and young adults, ages 10-21, and their families.
• Carry a regular caseload of 15-25 gang-involved program participants on an ongoing basis.
• Provides intense case management including home visits and transporting participants.
• Link these individuals to services and support.
• Help youth achieve educational success and employment.
• Assist youth with enrolling in school and maintaining attendance.
• Coordinate with partner agencies to support participants.
• Document client outcomes and contacts weekly.
• Participate in group sessions and related activities.
