Qualifikationen

• Associate degree in Human Services and 1-2 years experience working with youth/young adults.
• Valid driver's license and access to a vehicle on a continual basis.
• Experience providing holistic case management services to youth/young adults.
• Knowledge of Eastern Panhandle Social Services networks.

Aufgaben

• Conduct screenings and intakes.
• Engage in regular outreach to homeless youth.
• Analyze youth needs and resources.
• Complete needs assessments for all eligible youth.
• Assist young adults in developing goals and objectives to formulate action plans.
• Provide case management services including advocacy and referrals.
• Locate and refer clients to safe and affordable housing.
• Maintain accurate and timely case records, reports, and data.
• Interact with low-income populations.
• Maintain confidentiality within and outside of the agency.
• Work independently using initiative, creativity and flexibility.
