Job summary

Necco is seeking an Independent Living Case Manager/Life Coach to guide young adults toward independence across office, community, employment, and school settings.

You will develop and implement treatment plans, coordinate with schools and community resources, and ensure compliance with state and licensure standards while providing practical support for housing, safety, and daily living skills.

Qualifications

• Bachelor's degree in a human services field or related discipline.
• LSW or LPC licensure is preferred.
• Minimum 2 years’ experience with young adults having emotional/behavioral challenges.
• Must hold a valid Driver’s License and driving record in good standing.

Responsibilities

• Participate in referral matching for young adults in independent settings.
• Prepare apartments for intake, including furniture and basic necessities.
• Develop Initial and Comprehensive Treatment Plans for each client.
• Complete assessments (Biopsychosocial, Independent Living, Diagnostic, etc.).
• Ensure safety plans and initial legal documents are obtained prior to service delivery.
• Develop and monitor Individualized Treatment Plans (ITP) and track progress.
• Coordinate with schools, community resources, and placement agencies.
• Maintain confidentiality and comply with licensure and accreditation standards.
