Qualifications

• Minimum of 21 years of age.
• Current LSW or LPC or bachelor’s degree in a human services-related field.
• Minimum 2 years’ experience working with young adults with emotional or behavioral problems.
• Valid Driver’s License.
• Limited Liability Auto Insurance Coverage of 100/300/100.
• Organizational, and Written/Oral Communications skills.
• Ability to navigate software (MS Office, EHR, dashboards, databases).

Responsibilities

• Participate in referral matching process for young adults.
• Shop for and prepare apartments for intake (leases, furniture, basics).
• Facilitate Initial and Comprehensive Treatment Plans.
• Facilitate/ensure completion of biopsychosocial, independent living, diagnostic assessments.
• Ensure initial legal documentation is obtained prior to placement.
• Develop and monitor safety plans during intake.
• Develop and facilitate Individualized Treatment Plans (ITPs).
• Monitor implementation of treatment plans and progress.
• Meet/exceed state/licensure regulations for caseload contact.
• Liaise between Necco, placement agencies and community resources.
• Deliver services in various settings and travel as needed.
• Complete apartment inspections and address work orders.
• Participate in service meetings with schools, state workers, clinicians.
• Maintain confidentiality per policy.
