Job summary

Intercept Health is seeking a Life Skills Coach/Case Manager for its Young Adult Independent Living Program in Norfolk, VA. The role supports residents aged 17–21 with money management, employment, education, and daily living skills, guiding transition to community life.

The candidate will develop and implement transition plans, assist with transportation and admin duties, and engage as a positive role model in a structured program.

Qualifications

• Bachelor's degree in any field plus two years of experience in providing case management services to children and families.
• Or a Human Service Bachelor's degree with experience providing case management services to children/youth and their families preferred.
• Or a Human Service Master's degree with a student placement providing case management services to children/youth and their families.

Responsibilities

• Actively engage with young adults ages 17–21 preparing for transition into the community.
• Serve as a positive role model to observe and learn socially effective values, attitudes, and behaviors.
• Support residents in developing money management skills, obtaining employment, completing education as applicable, learning daily living skills, and exploring career development opportunities.
• Contribute to the formulation and implementation of the resident transitional living plan and assist with life skills learning, transportation, and administrative duties.

As a Full-time Life Skills Coach at YouthQuest's Independent Living Apartment Program, you will actively engage with young adults, ages 17-21, who are preparing for successful transition into the community. You will serve as a positive role model for residents to observe and learn socially effective values, attitudes, and behaviors. You will support residents in developing money management skills, obtaining employment, completing their education as applicable, learn daily living skills, and explore career development opportunities. Education and Experience Required: The selected candidate will have a Bachelor's diploma in any field plus two years of experience in providing case management services to children and families; OR a human service Bachelor's degree (Social Work, Psychology, Counseling, Special Education, etc) with experience providing case management services to children/youth and their families preferred; OR a human service Master's degree (Social Work, Psychology, Counseling, Special Education, etc) with a student placement providing case management services to children/youth and their families.
