Job summary

Intercept Health is seeking a Life Skills Coach/Case Manager for the Young Adult Independent Living Program in Virginia. You will guide residents aged 17–21 in developing life skills, budgeting, job readiness, and educational goals to prepare for community transition.

The role offers a $2,000 sign-on bonus and a $21–$23 hourly wage with full-time benefits, including health/dental/vision, retirement, and paid time off.

Qualifications

• Bachelor's diploma in any field plus two years of experience in providing case management services to children and families; OR a human service Bachelor's degree with experience providing case management services to children/youth and their families preferred; OR a human service Master's degree with a student placement providing case management services to children/youth and their families.

Responsibilities

• Actively engage with young adults aged 17–21 in Independent Living to model healthy values and behaviors.
• Help residents develop money management, employment, education, and daily living skills.
• Support career development opportunities and assist with transportation and administrative duties.
• Contribute to the resident transitional living plan and life skills learning.

As a Full-time Life Skills Coach at YouthQuest's Independent Living Apartment Program, you will actively engage with young adults, ages 17-21, who are preparing for successful transition into the community. You will serve as a positive role model for residents to observe and learn socially effective values, attitudes, and behaviors. You will support residents in developing money management skills, obtaining employment, completing their education as applicable, learn daily living skills, and explore career development opportunities. You will contribute to the formulation and implementation of the resident transitional living plan and assist with life skills learning, transportation, and administrative duties.

Education and Experience Required: The selected candidate will have a Bachelor's diploma in any field plus two years of experience in providing case management services to children and families; OR a human service Bachelor's degree (Social Work, Psychology, Counseling, Special Education, etc) with experience providing case management services to children/youth and their families preferred; OR a human service Master's degree (Social Work, Psychology, Counseling, Special Education, etc) with a student placement providing case management services to children/youth and their families.
